Q1: Can you please email me a return policy form, I did not get one for this product(lumican oxygen 3 filter).

A: The invoice that shipped with your filter has a return label on it. However, you don't need that if you no longer have it. Just box up the filter and include a note giving your order number and indicating the reason for the return, as well as whether you are returning for refund or exchange.
